Required Waiver – 2026–2027 Season
Medicine Hat Minor Hockey Association is asking all applicable families to complete the required waiver form for the 2026–2027 hockey season.
Please take a few minutes to review and complete the waiver at your earliest convenience. Your cooperation will help ensure all required paperwork is completed prior to the season.
